### SUMMARY OF POSITION

Intern Masters Level intern (social work/counseling), will work collaboratively with the discharge planner and CSU/WIC teams to develop comprehensive discharge plans for Clients admitted into the CSU.

Communicates clearly and appropriately; provides excellent customer service. Adheres to applicable policies and procedures. Regular attendance and punctuality is required.

### ESSENTIAL DUTIES & RESPONSIBILITIES

Meets one-on-one with each Client in CSU/WIC to collaboratively develop a discharge plan that addresses that Client's presenting problem/primary needs:

- Mental health: Outpatient appointment scheduling (psychiatric/counseling), safety planning, restricting access to lethal means, connection to follow-up programs
- Physical health: PCP resources and appointments as needed
- Substance Use: Relapse prevention plan, treatment resources and referrals as needed
- Housing: Resources and referral assistance as needed
- Financial, Legal, Social: Resources and referrals as needed

### KNOWLEDGE, SKILLS & ABILITIES

- Effectively communicate via written, verbal, in person and virtual methods.
- Perform basic electronic medical record computer skills
- Detail oriented with effective problem solving skills
- Knowledge base in community resources
- Skill in crisis intervention
- Ability to de-escalate patient/clients using evidence based de-escalation training (e.g. Theapeutic Options) to include safety holds as necessary.

Bachelor's degree required. Enrolled in a Masters Level Program.
